Waiver and Release Form for Conectamente Hypnosis Experiences:

I, the undersigned, hereby voluntarily wish to participate in the Conectamente Hypnosis Experience hosted by  Lilia Tome/Talent Unlocked, LLC. I understand that the activities involve guided hypnosis and mental exercises facilitated by certified professionals.

Assumption of Risk: I acknowledge that participation in hypnosis activities primarily involves risks related to emotional and psychological responses. I understand that while the activities are generally very safe and conducted in a controlled environment by certified professionals, emotional discomfort or unexpected psychological reactions can occur. I affirm that I am voluntarily participating in these activities with knowledge of the potential emotional responses involved and agree to assume any and all risks related to such responses.

Exclusion of Participants: This event is not suitable for individuals who have been diagnosed with psychotic conditions, are on the autism spectrum, or are under the influence of alcohol or drugs. By signing this waiver, I affirm that I do not fall into these categories.

Medical Acknowledgement: I declare that I am physically and mentally fit to participate in these activities and have no medical condition that would affect my safety or health by participating. I will notify the facilitator of any known medical conditions or medical treatment I am undergoing that may affect my participation.

Media Release: I consent to the use of my image or video taken during the event in promotional materials and publications. I waive the right to inspect or approve the finished product, wherein my likeness appears.

Waiver of Liability: I hereby release, waive, discharge, and covenant not to sue Lilia Tome, Talent Unlocked, LLC, its officers, employees, agents, team members, and any collaborators or partners involved in the Conectamente Hypnosis Experiences, from liability from any and all claims resulting from the inherent risks of the activity or the ordinary negligence of the aforementioned parties

Acknowledgment of Understanding: I have read this waiver of liability, fully understand its terms, and acknowledge that I am signing it freely and voluntarily. My signature signifies my intention to provide a complete and unconditional release of liability to the greatest extent allowed by law.
