Participant Release, Waiver of Liability & Assumption of Risk
Please read carefully before participating.
1. Assumption of Risk
I understand that participation in mat Pilates classes, fitness events, and related activities hosted by Praise + Pilates HTX involves physical exertion and inherent risks, including but not limited to: muscle strains, sprains, joint injuries, cardiovascular events, and other physical harm. I voluntarily assume all such risks, whether known or unknown, and accept full responsibility for my participation.

2. Medical Clearance
I affirm that I am in good physical health and have no medical conditions, injuries, or physical limitations that would prevent safe participation in Pilates or fitness activities. I have consulted with a physician if I have any concerns about my ability to participate. I agree to inform the instructor of any injuries, limitations, or health concerns before each class.

3. Release of Liability
In consideration of being allowed to participate in Praise + Pilates HTX classes and events, I hereby release, waive, discharge, and covenant not to sue Praise + Pilates HTX, its owner(s), instructors, volunteers, vendors, and venue partners (collectively, "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained by me during my participation, whether caused by the negligence of the Released Parties or otherwise.

4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur as a result of my participation, including any claims brought by third parties.

5. Photo & Video Consent
I grant Praise + Pilates HTX permission to photograph or record video during classes and events, and to use such images or footage for promotional purposes including but not limited to Instagram, Sweatpals, and other digital platforms. I understand I may opt out by notifying the host in writing before the class or event.

6. Class Policies
•	Please arrive 5–10 minutes before class. Latecomers may not be admitted once class has begun to avoid disruption.
•	Memberships and class packs are non-refundable. Class packs expire 60 days from the date of purchase.
•	Praise + Pilates HTX reserves the right to cancel or reschedule classes with reasonable notice.
•	Participants are responsible for their own equipment. Praise + Pilates HTX is not responsible for lost or stolen items.

7. Governing Law
This agreement shall be governed by the laws of the State of Texas. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

8. Acknowledgment
By checking the acknowledgment box, I confirm that I have read this waiver in full, understand its contents, and agree to its terms voluntarily and without coercion. I acknowledge that this is a legally binding document.
