Discolates® Class Waiver & Release of Liability
By registering for or attending a Discolates® class or event, I acknowledge that I have read, understood,
and voluntarily agree to the following:

1. Voluntary Participation & Assumption of Risk
I understand that participation in movement-based activities, including but not limited to Pilates, dance,
stretching, strength training, mobility exercises, wellness activities, and other fitness programming involves
inherent risks. These risks include, but are not limited to, muscle strains, sprains, falls, equipment-related
injuries, illness, heart-related events, or other physical injury.
I voluntarily choose to participate and assume full responsibility for any risks associated with my
participation.
I certify that I am physically able to participate in the activities offered and understand that it is my
responsibility to consult with a physician before participating if I have any medical conditions, injuries,
pregnancy, postpartum considerations, or other health concerns.

2. Health Acknowledgment
I acknowledge that I am solely responsible for determining whether I am physically able to participate in
this event.
If I am pregnant, postpartum, recovering from an injury or surgery, or have any medical condition or
physical limitation that may affect my ability to safely participate, I certify that I have consulted with an
appropriate healthcare provider or have independently determined that participation is appropriate for me.
I agree to modify movements as needed, stop participating immediately if I experience pain, dizziness,
faintness, shortness of breath, or any other concerning symptoms, and notify the instructor if I require
assistance.
I understand that Bianca Falcon, Discolates®, instructors, staff, volunteers, sponsors, collaborators, and
event partners are not responsible for determining whether I am medically fit to participate.

3. Release of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Bianca Falcon,
Discolates®, Before & After Sound Lab, all event venues, sponsors, collaborators, vendors, volunteers,
instructors, contractors, employees, and representatives from any and all claims, liabilities, demands,
damages, expenses, losses, or causes of action arising out of or related to my participation in the event,
except to the extent caused by gross negligence or intentional misconduct.
This release applies to any injury, illness, death, property damage, or other loss that may occur before,
during, or after the event.

4. Products, Samples, Gifts & Giveaways
I understand that products, samples, food, beverages, gifts, promotional items, or giveaways may be
distributed by Discolates® or event partners.
I acknowledge that it is solely my responsibility to review all ingredients, materials, warnings, and usage
instructions before consuming or using any item.
I voluntarily assume all risks associated with these products and release Bianca Falcon, Discolates®,
sponsors, vendors, collaborators, and event partners from any liability arising from allergic reactions,
sensitivities, injuries, illnesses, or other adverse outcomes related to their use.

5. Additional Event Activities
I understand that participation in any additional activities, including networking experiences, workshops,
performances, contests, activations, demonstrations, or other programming, is completely voluntary.
I choose to participate at my own risk and release all released parties from liability arising from my
participation.

6. Personal Property
I understand that I am solely responsible for my personal belongings. Discolates®, Before & After Sound
Lab, event venues, and event partners are not responsible for any lost, stolen, or damaged personal
property.

7. Photo & Video Release
I grant Discolates®, Bianca Falcon, and authorized event partners permission to photograph, film, record,
and otherwise capture my likeness during the event.
I understand these images and recordings may be used in print, digital media, websites, social media,
advertising, marketing, editorial content, educational materials, and future promotional campaigns without
compensation or additional permission.
I waive any right to inspect or approve the final materials in which my likeness appears.

8. Emergency Medical Care
If I become injured or require emergency medical treatment during the event, I authorize event staff to
seek emergency medical assistance on my behalf if I am unable to do so.
I understand that I am solely responsible for any medical expenses incurred and that Discolates® assumes
no responsibility for those expenses.

9. Alcohol & Third-Party Services
If alcoholic beverages or third-party services are offered during the event, I acknowledge that any decision
to consume alcohol or participate in such services is entirely voluntary and at my own risk.

10. Right to Refuse Participation
Discolates® reserves the right to refuse or discontinue participation for any individual whose behavior is
unsafe, disruptive, or poses a risk to themselves, other participants, instructors, staff, or the venue. Any
refund will be subject to the event's stated cancellation policy.

11. General Terms I understand that:
This Waiver & Release shall be governed by the laws of the State of Florida.
If any provision of this Waiver is determined to be invalid or unenforceable, the remaining provisions
shall remain in full force and effect.
This Waiver applies to all Discolates® programming, classes, and activities taking place during the
event.

Final Acknowledgment
Before submitting this waiver, I acknowledge that I have had the opportunity to ask questions regarding
this Waiver & Release of Liability and that any questions I had have been answered to my satisfaction.
By checking the required acknowledgment boxes, initialing each section, and submitting this waiver
through the registration platform, I acknowledge and agree that:

I have carefully read this entire Waiver & Release of Liability. I fully understand its contents and voluntarily agree to all of its terms. I understand that I am giving up certain legal rights, including the right to bring certain legal claims
against the released parties.

I certify that I am at least 18 years of age, or that my parent or legal guardian has completed this
waiver on my behalf.

I understand that my electronic acknowledgment, initials, and electronic signature submitted through the registration platform constitute my legally binding acceptance of this Waiver & Release
of Liability.
