Sparc Lab Liability Waiver & Media Release

By participating in any class, training session, workshop, or event hosted by Sparc Lab, you acknowledge and agree to the following:

1. Assumption of Risk

I understand that dance, fitness, and physical training activities carry inherent risks, including but not limited to slips, falls, muscle strains, sprains, collisions, and other injuries. I voluntarily assume full responsibility for any risks associated with my participation.

2. Release of Liability

I hereby release, waive, and discharge Sparc Lab, its owners, instructors, staff, affiliates, volunteers, and venue partners from any and all claims or liability for injuries, accidents, damages, or losses that may occur before, during, or after participation in Sparc Lab activities. This release applies to all claims, including those resulting from negligence, to the fullest extent permitted by law.

3. Fitness to Participate

I certify that I am physically capable of participating in dance and fitness activities. I understand I am responsible for monitoring my own physical condition and agree to stop participating if I feel pain, discomfort, dizziness, or unsafe in any way. I acknowledge that it is my responsibility to consult with a healthcare provider if needed.

4. Personal Property

I understand that Sparc Lab is not responsible for lost, stolen, or damaged personal belongings.

5. Media Release (Automatic by Attendance)

I acknowledge that by attending any Sparc Lab class or event, I grant Sparc Lab full permission to record, photograph, and capture video of me during participation. I authorize Sparc Lab to use these images and recordings for marketing, promotional, educational, and commercial purposes in any format, without compensation. I waive any rights to inspect or approve the final materials.

6. Acknowledgment & Agreement

By signing below, I confirm that I have fully read, understand, and agree to this Liability Waiver & Media Release. I acknowledge that my participation is voluntary and that Sparc Lab is not liable for any injuries or damages that may occur.
