Participant acknowledges that any physical improvement is the result of the physical and mental efforts
put forth and that any physical improvement will vary from individual to individual. Participant agrees
that none of the Company and their affiliates nor any employee, agent or representative of any of them
has made any representation, claim or guarantee that the participant will realize any physical
improvement as a result of this agreement or the services provided By LH Fitness Solutions.
Participant acknowledges that exercise; use of the equipment, rented space, and facilities of LH Fitness
Solutions and other affiliates naturally involves the risk of injury and medical disorders, including death,
whether member, someone else, some activity or something causes it. Participant agrees that the
participant engages in all exercise, competition and other activities operated, provided, organized,
arranged or sponsored but any of the company (LH Fitness Solutions) and their affiliates, either on or off
the facility’s premises, and uses all facilities and services of LH Fitness Solutions and affiliates at such
persons own risk. This is including but not limited to restrooms, rented facilities, equipment, parking lot,
and studios. LH Fitness Solutions assumes no liability for health hazards, illness, and death created from
allergic reactions to foods, supplements, and nutrition suggested. Each participant is responsible for
knowing their allergens and should consult their doctor before entering this and any nutritional and or
exercise program. LH Fitness Solutions assumes no liability for health hazards, illness, injury, and death
created from pre-existing physical and health conditions that worsen as a result of services rendered.
Individuals are responsible for understanding pre-existing conditions and the risks associated.
Participant shall hold LH Fitness Solutions and affiliates harmless from any and all loss, claim, injury,
damage, and liability sustained or incurred by members resulting from or arising out of the negligent acts
and omissions and any other acts and omissions, and alleged acts and omissions and other acts and
omissions of any staff member of LH Fitness Solutions, affiliates, and rented facilities. LH Fitness
Solutions assumes no liability for injury including death for defective equipment and machinery used.
Participants agree to use this equipment at their own risk. Any and all activities engaged with LH Fitness
Solutions are agreed to be completely voluntary and at the risk.
