Thank you for joining us for this Pilates experience. Before participating, all attendees must read and sign the following waiver and release of liability.


PARTICIPANT WAIVER AND RELEASE OF LIABILITY

I, the undersigned, acknowledge and agree to the following:

1. Voluntary Participation

I am voluntarily participating in the Pilates Rooftop Event hosted by Balance Social Club. I understand that physical exercise, including Pilates, involves inherent risks, including but not limited to muscle soreness, strains, sprains, and risk of injury.

2. Health and Physical Condition

I represent that I am physically fit and have no medical condition that would prevent my full participation in this event. I agree to listen to my body, rest as needed, and notify the instructor or host of any discomfort.

3. Assumption of Risk

I understand that participating in outdoor rooftop fitness activities comes with added risks, including but not limited to weather conditions, uneven surfaces, and the possibility of falls or accidents. I assume all risks associated with participating in this activity.

4. Waiver and Release

I hereby release, waive, discharge, and hold harmless Balance Social Club, its owners, hosts, collaborators, partners, event instructors, and the AZUL venue and staff from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or damage that may be sustained while participating in this event.

5. Media Release (Optional)

I give permission for photos/videos taken during the event to be used for promotional purposes by Balance Social Club on social media or marketing platforms.
