1. Event and activities
I voluntarily choose to participate in Finding You: Sunset Yoga, Sculpt & Glow, hosted by BODY The
Movement Society on October 25, 2026, at 7:00 p.m. at Café ZunZun, Texas (the Event). Activities include
light yoga, mat Pilates, sculpt, stretching, bodyweight exercises, use of any supplied fitness equipment, and
related onsite event activities.
2. Parties covered by this agreement
The "Released Parties" are the legal owner/operator of BODY The Movement Society identified above; Monica
Blouin, individually and as event organizer/instructor; instructors engaged to teach this Event; Event
employees and volunteers; and the legal owner/operator of Café ZunZun identified above, together with its
employees and agents acting in connection with the Event.
3. Risks and voluntary assumption of risk
I understand that physical activity carries risks, including muscle or joint strains, falls, dizziness, dehydration,
equipment-related injuries, and serious injury or death. Outdoor evening conditions may include heat, weather
changes, insects, uneven or slippery surfaces, and limited lighting. Injuries may occur even with modifications
or supervision. I knowingly and voluntarily assume the risks of my participation, including known and unknown
risks and risks arising from the ordinary negligence of the Released Parties, subject to the exclusions in
Section 5.
4. Health, modifications and safe participation
I will consider my health and ability before participating and seek medical advice if I am uncertain whether
these activities are appropriate for me. I understand this Event provides fitness instruction, not medical
treatment. I will tell my instructor about limitations relevant to safe participation, follow safety instructions, use
equipment as directed, and stay within my abilities. I may decline any exercise, request a modification, rest, or
stop at any time. I will stop and alert an instructor if I experience pain, dizziness, faintness, or other concerning
symptoms. I will inspect my exercise area and equipment and report hazards. I will not participate while
impaired by alcohol or drugs.

5. RELEASE OF LIABILITY - INCLUDING ORDINARY NEGLIGENCE
TO THE EXTENT PERMITTED BY TEXAS LAW, I RELEASE THE RELEASED PARTIES FROM CLAIMS
FOR PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE ARISING FROM MY PARTICIPATION IN
THE EVENT, INCLUDING CLAIMS CAUSED IN WHOLE OR IN PART BY THE ORDINARY NEGLIGENCE
OF ANY RELEASED PARTY, WHETHER SOLE OR CONCURRENT, INCLUDING NEGLIGENT
INSTRUCTION, SUPERVISION, EQUIPMENT MAINTENANCE, OR MAINTENANCE OF THE EVENT AREA.
THIS RELEASE DOES NOT APPLY TO GROSS NEGLIGENCE, RECKLESSNESS, INTENTIONAL
MISCONDUCT, OR LIABILITY THAT CANNOT LAWFULLY BE WAIVED.
6. Emergency assistance
If I am injured or become ill, I authorize Event personnel to call emergency services and seek appropriate
assistance on my behalf. This authorization does not require anyone to provide medical treatment beyond their
training or promise that medical personnel will be onsite. I understand that I am responsible for medical
expenses as applicable under law and my insurance coverage.
7. Texas law and severability
Texas law governs this agreement. If a provision is found unenforceable, the remaining provisions will continue
to apply to the extent permitted by law.
8. Adult acknowledgment and signature
I confirm that I am at least 18 years old. I have read this entire agreement, understand that it releases certain
legal rights, including claims for the Released Parties' ordinary negligence, and sign voluntarily. I have had an
opportunity to ask questions. This form is for adult participants only. My signature applies to Sections 1 through
8; photo/video permission below is a separate, optional choice.
