WAIVER AND RELEASE OF LIABILITY AGREEMENT
Transfiguration Fitness LLC

This Waiver and Release of Liability (“Agreement”) is entered into by the participant (“Client”) in favor of Transfiguration Fitness LLC, its owner Geovannie Valdez, all affiliated independent contractors, agents, representatives, employees, trainers, and event partners (collectively referred to as “Released Parties”).

By participating in this class and reviewing this document, I acknowledge, agree, and understand the following:

1.	Assumption of Risk
I understand that participating in fitness classes, personal training, physical assessments, and recovery services involves inherent physical risks, including but not limited to: injury, muscle strain, fatigue, heart complications, dehydration, or serious harm. I voluntarily assume all risks, known and unknown, related to my participation.

2.	Release of Liability
In consideration for being allowed to participate in any activities conducted or coordinated by Transfiguration Fitness LLC and its fitness specialists, I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all claims, demands, causes of action, or liability resulting from injury, illness, death, or property damage arising from my participation.

3.	Medical Clearance
I confirm that I am physically and medically able to engage in physical activity. I agree to notify Transfiguration Fitness LLC and its fitness specialists of any health conditions that may affect my ability to participate safely.

4.	Independent Contractors
I acknowledge that some services are provided by independent contractors and not employees of Transfiguration Fitness LLC. I understand that Transfiguration Fitness LLC is not liable for the acts or omissions of independent contractors beyond the scope of the company’s written agreements.

5.	Photo & Video Release
I grant Transfiguration Fitness LLC permission to use any photos or videos taken during sessions for promotional purposes unless I request otherwise in writing.

6.	Cancellation Policy
I agree to adhere to the cancellation and no-show policies as outlined in my membership or training agreement.

7.	Severability
If any portion of this Agreement is found to be unenforceable, the remaining terms shall remain in full force and effect. I confirm that I have read, understood, and voluntarily agree to the terms of this Waiver and Release of Liability.
