Pickleball

WAIVER AND RELEASE OF LIABILITY

In consideration of being permitted to participate in the Pickleball event organized by PicklePals, I, the undersigned, acknowledge and agree to the following:

1. Assumption of Risk: I understand and acknowledge that participation in Pickleball involves physical activity and carries inherent risks, including but not limited to falls, contact with other participants, and injuries resulting from the sport or facility conditions. I voluntarily assume all such risks.

2. Release and Waiver: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless [Organizer Name], its owners, officers, employees, volunteers, sponsors, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained while participating in the event.

3. Medical Treatment: I give permission for the event organizers to provide emergency medical treatment as deemed necessary. I understand that I am responsible for all medical costs incurred as a result of my participation.

4. Photographic Release: I grant [Organizer Name] permission to use photographs and videos taken during the event for promotional purposes, without compensation or further approval.

5. Fitness to Participate: I certify that I am physically fit and have not been advised otherwise by a qualified medical professional.

BY ATTENDING THE EVNET, I AFFIRM THAT I HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE, AND I SIGN IT VOLUNTARILY.
