INJURY/LIABILITY RELEASE AND WAIVER OF CLAIMS FORM
MAN/kāv and CrossFit Coconut Grove

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS.

In consideration of being allowed to participate in activities, workouts, or events organized by MAN/kāv and/or CrossFit Coconut Grove (collectively referred to as the “Organizations”), I hereby acknowledge and agree as follows:

1. Assumption of Risk

I understand and acknowledge that:
	•	Participating in fitness activities, including but not limited to CrossFit, weightlifting, cardiovascular exercises, and any other physical activity, carries inherent risks of injury, illness, or even death.
	•	Risks include, but are not limited to, muscle strains, sprains, fractures, heart attacks, dehydration, and other medical conditions or injuries.
	•	I voluntarily choose to participate in these activities and assume full responsibility for any risks, known or unknown, that may result from my participation.

2. Release and Waiver of Liability

I hereby release, waive, and forever discharge MAN/kāv, CrossFit Coconut Grove, their owners, trainers, coaches, employees, volunteers, agents, representatives, and affiliated entities (collectively referred to as the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss I may suffer during or as a result of participation in the activities.

This waiver includes, but is not limited to, claims arising from:
	•	Negligence by the Released Parties.
	•	Dangerous or defective equipment or property.
	•	Improper supervision or instruction.

3. Medical Consent

I represent that I am physically fit and have no medical condition that would prevent me from safely participating in fitness activities. I agree to stop participating immediately and notify a representative of the Organizations if I experience any pain, discomfort, or injury.

In the event of a medical emergency, I authorize the Released Parties to seek emergency medical treatment on my behalf and accept financial responsibility for any resulting costs.

4. Indemnification

I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, damages, or costs, including attorney’s fees, arising out of my participation in the activities or my failure to comply with the terms of this agreement.

5. Photo/Video Release 

I grant permission for my photo, video, or likeness to be used in promotional materials by the Organizations.

6. Binding Agreement

I understand that this agreement is binding upon me, my heirs, executors, administrators, and assigns, and that it will be governed by the laws of the State of Florida.

7. Acknowledgment of Understanding

I have read this release and waiver in its entirety, fully understand its terms, and voluntarily agree to be bound by them. I acknowledge that I am giving up legal rights, including the right to sue, by signing this document.
