WAIVER & RELEASE OF LIABILITY

By registering for and participating in this event, I acknowledge that martial arts, fitness training, and related physical activities involve inherent risks that cannot be eliminated regardless of the level of supervision or safety precautions.

I understand that participation may involve physical contact, rapid movements, falls, striking drills, defensive techniques, stretching, conditioning exercises, and other activities that may result in discomfort, soreness, muscle fatigue, bruises, sprains, strains, cuts, fractures, concussions, dislocations, permanent disability, or, in rare cases, serious injury or death.

I understand that it is normal to experience temporary muscle soreness, stiffness, fatigue, or minor aches following physical activity. However, I agree to immediately stop participating and notify an instructor if I experience severe pain, dizziness, shortness of breath, chest pain, numbness, loss of balance, or any symptoms that could indicate a serious medical condition.

I certify that I am in good physical condition and capable of participating in this event. I understand that it is my responsibility to consult with a physician before participating if I have any medical conditions, injuries, physical limitations, or concerns that could affect my ability to safely participate.

I voluntarily assume full responsibility for all risks, known and unknown, associated with my participation, including those resulting from my own actions, the actions of other participants, the condition of the facility or equipment, or the ordinary negligence of The Wellness Kickback, the host facility, instructors, volunteers, sponsors, partners, and affiliates, to the fullest extent permitted by law.

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless The Wellness Kickback, the host venue, instructors, employees, volunteers, sponsors, partners, and affiliates from any and all claims, demands, damages, liabilities, costs, or causes of action arising out of or related to any injury, illness, loss, or damage sustained during or resulting from my participation.

I authorize emergency medical treatment if I become unable to provide consent during an emergency and understand that I am solely responsible for any medical expenses incurred.

I also grant permission to The Wellness Kickback to photograph, film, or otherwise record my participation and to use my name, image, voice, and likeness for promotional, marketing, educational, and social media purposes without compensation.

By completing my registration, I acknowledge that I have carefully read this waiver, fully understand its contents, understand that I am giving up certain legal rights, and voluntarily agree to its terms.
