I voluntarily choose to participate in this Pilates class led by Marissa Maldonado at Powerhouse Gym Miami and assume full responsibility for any and all injuries, losses, or damages that may occur.
I release and hold harmless Marissa Maldonado, Powerhouse Gym Miami, and all staff, employees, and affiliates from any and all liability, claims, or demands related to personal injury, illness, property damage, or other loss that may arise during or as a result of my participation.

No Refund Policy
I acknowledge that all ticket sales are final and non-refundable under any circumstance, including but not limited to no-shows, scheduling conflicts, or dissatisfaction with the class.

Photo & Video Release
I give full consent to be photographed and/or filmed during the class for promotional or commercial use by Marissa Maldonado and Powerhouse Gym Miami. I understand that these images and videos may be used on social media, websites, and marketing materials, and I waive any rights to royalties or compensation for their use.
