 Participant Liability Waiver & Media Release Agreement
 Effective as of the date of signature or digital agreement.

PLEASE READ CAREFULLY BEFORE PARTICIPATING. BY SIGNING OR OTHERWISE INDICATING YOUR AGREEMENT, YOU ARE WAIVING CERTAIN LEGAL RIGHTS.
1. Assumption of Risk
By signing up for and participating in this Pilates class hosted by Pi·Lattes LLC, I acknowledge and agree to the following:
I understand that participation in physical fitness activities such as Pilates involves inherent risks, including but not limited to muscle strains, sprains, falls, or other injuries. I affirm that I am in good physical condition and capable of safely engaging in this activity. I accept full responsibility for my personal health and well-being during the class and agree to immediately cease participation and seek assistance if I experience discomfort, dizziness, or pain.

2. Waiver and Release of Liability
I hereby voluntarily waive, release, discharge, and hold harmless Pi·Lattes LLC, its instructors, contractors, employees, representatives, venue partners, and hosts from any and all liability, claims, demands, or causes of action, whether known or unknown, arising out of or related to any injury, illness, damage, or loss that I may sustain as a result of my participation in this class, regardless of whether such injury or loss is caused by negligence or otherwise.

3. Media Release Consent
I understand that this class may be photographed and/or recorded for promotional and marketing purposes. I hereby grant permission for Pi·Lattes LLC and its affiliates to use any photographs, video recordings, or other media featuring my likeness in promotional materials, including but not limited to social media platforms (e.g., Instagram, TikTok), websites, and printed materials, without compensation.
If I do not wish to be photographed or recorded, I understand that it is my responsibility to notify the instructor prior to the start of class.

4. Age Requirement
By participating, I confirm that I am at least 18 years of age.
If I am under the age of 18, I understand that I must be accompanied by a parent or legal guardian who must also sign this waiver on my behalf. Unaccompanied minors are not permitted to participate.

5. Acknowledgment and Acceptance
By signing below (or checking the acknowledgment box if signing electronically), I acknowledge that I have read, understood, and agree to the terms and conditions of this Participant Liability Waiver & Media Release Agreement. I enter into this agreement voluntarily and with full knowledge of its legal significance.
