**Pilates for PACE: A Birthday Charity Event**

This year, I’m celebrating my birthday in the best way possible, by giving back! *Pilates for PACE* is a special charity Pilates class where every move makes a difference. In partnership with board member Priscilla Lauture, all proceeds from this event will go directly to the **PACE Center for Girls**, an incredible organization dedicated to helping young women through education, counseling, and the support they need to build brighter futures.

Come ready to sweat, strengthen, and flow in a fun, all-levels Pilates class led by me.

After class, we’ll celebrate my birthday together, knowing that your participation is making an impact in the lives of girls right here in our community. Together, we can empower the next generation!

Not ready to join the class? You can still be part of the impact! Grab a spectator pass and cheer us on while supporting the cause, or choose an extra giving tier if you’d like to make an additional donation.

LIABILITY WAIVER AND RELEASE FORM

HEALTH AND FITNESS ACKNOWLEDGMENT
By signing this waiver, I acknowledge that:

I am in good physical condition and have no medical conditions that would prevent my participation in physical exercise.

I have consulted with a physician if I have any concerns about my ability to participate in this fitness activity.

I understand that Pilates involves physical exertion and stretching that may be strenuous.

I will inform the instructor immediately if I experience any pain, discomfort, or unusual symptoms during the class.

I will notify the instructor if I have any injuries, medical conditions, or physical limitations.


ASSUMPTION OF RISK
I understand and acknowledge that:

Participation in physical exercise activities, including but not limited to Pilates, stretching, and movement to music, involves inherent risks of injury.

These risks include, but are not limited to: muscle strains, sprains, fractures, cardiac episodes, and other bodily injuries.

The use of equipment, mats, and props during the class may present additional risks.

I am voluntarily participating in this activity with full knowledge of these risks.

I assume full responsibility for any and all risks of injury or damage that may occur during my participation.


RELEASE AND WAIVER OF LIABILITY

In consideration for being allowed to participate in the Pilates for PACE event, I hereby:

RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE:

Pilates with Heather and its instructors
1920 YBor and its owners/operators
PACE
Event organizers, sponsors, and volunteers

Any other parties associated with this event:

From any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in this activity.

INDEMNIFICATION
I agree to INDEMNIFY AND HOLD HARMLESS the parties listed above from any loss, liability, damage, or costs they may incur due to my participation in this event, whether caused by negligence or otherwise.

MEDIA RELEASE
I grant permission for photographs, video recordings, or other media taken during this event to be used for promotional purposes by Pilates with Heather, PACE, and 1920 Ybor.

AGREEMENT TERMS

This waiver shall be binding upon my heirs, estate, executors, and administrators.
This agreement is governed by the laws of the State of Florida.

If any portion of this agreement is deemed invalid, the remainder shall remain in full force.
I have read this entire waiver and understand its contents.

I am signing this agreement voluntarily and of my own free will.


I HAVE READ THIS WAIVER AND RELEASE, UNDERSTAND ITS TERMS, AND SIGN IT VOLUNTARILY.
