Liability Waiver & Release of Claims

1. Assumption of Risk
I understand that participation in this event includes activities such as Pilates/fitness instruction, wellness services (including but not limited to B12 injections, IV therapy, and wellness shots), bracelet making, vendor shopping, and general event participation.
I acknowledge that physical activity and wellness services involve inherent risks, including but not limited to muscle strain, injury, allergic reaction, dizziness, adverse reactions to injections or IV therapy, or other unforeseen medical complications.
I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.

2. Medical Acknowledgment
I certify that:
	•	I am physically able to participate in Pilates and related movement activities.
	•	I do not have any medical conditions that would prevent safe participation.
	•	I understand that B12 injections, IV therapy, and wellness shots may carry risks, and I will disclose any allergies, medical conditions, or medications to the licensed provider administering services.
	•	I understand that wellness providers are independent vendors and responsible for their own services.
If I am pregnant, nursing, have a medical condition, or am under medical care, I acknowledge that I am participating at my own discretion.

3. Release of Liability
In consideration of being permitted to participate, I hereby release, waive, and discharge the event host(s), The Bamboo Grove, event organizers, instructors, vendors, wellness providers, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of any injury, illness, loss, or damage related to participation in this event.

4. Photo & Media Release
I grant permission for photographs and videos taken during the event to be used for promotional and marketing purposes unless I notify the event host in writing prior to the event.

5. Indemnification
I agree to indemnify and hold harmless the event host(s), venue, instructors, and vendors from any claims resulting from my participation.

6. Agreement
I have read this waiver carefully and fully understand its contents. I voluntarily agree to its terms.
