Turn Up Dance Fitness with Jori - Liability Waiver & Release

By participating in any class, event, workshop, or activity offered by Turn Up Dance Fitness with Jori (the “Activity”), I acknowledge and agree to the following:


Assumption of Risk
I understand that dance and fitness activities involve physical movement and may include risks of injury, illness, or other complications. I voluntarily choose to participate and accept full responsibility for my own well-being, including any injury, illness, or property damage that may result from my participation.


Release of Liability
I release, waive, and discharge Turn Up Dance Fitness, Turn Up Dance Fitness with Jori, all hosting studios/venues, instructors, assistants, staff, and affiliates (“Released Parties”) from any and all claims, demands, or causes of action that arise out of or relate to my participation in the Activity, including claims caused in whole or in part by the negligence of the Released Parties.


Health Confirmation & Personal Responsibility
I confirm that I am physically able to participate in this activity and have disclosed any health conditions that may affect my safety. I understand that I am responsible for listening to my body, modifying movements as needed, and stopping if I feel pain, dizziness, or discomfort. I acknowledge that instructors provide general guidance and are not responsible for assessing my fitness level or physical limitations.


Emergency Medical Authorization
In the event of an emergency, I authorize Jori Bloom and any instructor or representative to seek necessary medical care on my behalf. I understand that I am responsible for any associated costs.


Media Release
I grant permission for photos or videos taken during the Activity to be used for promotional or educational purposes in any media format and waive my any right to compensation.


Acknowledgment
I have read and understand this waiver. I am signing it voluntarily. I understand that this document affects my legal rights and that I may wish to consult an attorney before signing.
