1. Acknowledgment of Participation

I voluntarily choose to participate in the Upleveled Gratitude & Move event, which includes physical movement, Pilates, and guided mindset activities. I understand that participation involves physical exertion and that it is my responsibility to consult a medical professional if I have any concerns about my health or ability to participate.

2. Assumption of Risk

I understand that participating in physical activity carries certain inherent risks, including but not limited to injury, strain, fatigue, or exposure to environmental conditions. I knowingly and voluntarily assume all such risks associated with participating in this event.

3. Release of Liability

In consideration of being allowed to participate, I release and hold harmless Upleveled Wellness, Move Pilates, Visit Tucson, and their owners, instructors, staff, volunteers, and affiliates from any and all claims, liabilities, or demands arising from my participation in the event. This release applies to any injury, illness, or loss that may occur before, during, or after the event.

4. Media Release

I acknowledge that this event will be photographed and filmed by Visit Tucson and may be used by Upleveled Wellness, Move Pilates, and Visit Tucson for promotional, marketing, and social media purposes. I grant permission for my image, likeness, and voice to be captured and used without compensation or further approval.

5. Health & Safety

I affirm that I am not experiencing any symptoms of illness and will follow all event health and safety guidelines. I understand that participation is voluntary and at my own discretion.
